We break out the old slab, haul it off, and look at what's underneath, because that's usually where the trouble started. Then we rebuild the base: dig out the soft ground, bring in gravel, compact it, set steel, and pour new concrete sloped so water runs away from the house, with control joints cut at the right spacing and a proper cure.
